Northern Colorado Hospitalists is an independently owned physician practice founded in 2004, now employing over 60 physicians with a focus in adult medicine. We promote a culture of service, quality and collaboration in an effort to provide the best care possible to our community. Our acute care division currently provides services at Poudre Valley Hospital, Medical Center of the Rockies, and UCHealth Greeley Hospital.
After almost 20 years of service, NCH understands the increasing challenges of navigating care once discharged from the hospital setting. As the medical system has become more complex, we recognize the need for innovation during these transitions to improve patient success to return and remain at home. Our new post-acute care division, NCH Health Partners, is a collaborative program developed to meet these needs upon discharge from the hospital to facility-based rehab or home.
Our goal is to help navigate the bridge to health and independence.
We aim to collaborate with your primary care team to ensure safe transitional care as you move through your healthcare journey.
If you are a long-term care or assisted living resident, we also recognize the value of being seen in your home.
We aim to provide primary services in this setting with a focus on quality of life and goal directed care.
The team at NCH Health Partners is geriatrician led with an understanding in managing complex chronic illness and life with disability. Our team understands the various levels of care and the unique challenges that aging can bring. While we all come from various backgrounds, our passion is delivering quality care that is patient centered to meet your goals of care.
