Northern Colorado Hospitalists

About Northern Colorado Hospitalists

Northern Colorado Hospitalists was founded in 2004 by 3 Hospitalists that joined forces to provide inpatient care at Poudre Valley Hospital. Since that time, the group has grown to over 60 physicians with two additional UC Health hospitals, Medical Center of the Rockies in 2007 and Greeley Hospital in 2019. Two of our founding partners are still with the group, 18 years later.

NCH Health Partners was founded in 2023 in order to provide care for patients in the post acute care setting.  We partner with multiple Skilled Nursing and Long Term Care facilities, Assisted Living Facilities, and Acute Rehab facilities, as well as assist with transition of care, in order to provide seamless, high quality care.
